This form is for a Stallholder Application for SelbyFest 2024 at Selby Community House. Once submitted, the form will be reviewed for approval and we will notify you ASAP whether your application has been accepted.

Stall sizes are 3m X 3m. Please bring your own gazebo/ shelter for weather protection, and your own table.

Do you have your own insurance? If YES, attach certificate of current policy (right-hand section). If NO, you will be required to pay $15 to be included under Selby Community House Inc.'s Public Liability Insurance.

Child Safety Regulations require all Stallholders conducting 'child participation activities' at their stall, to hold a current WWC check. For more information: www.workingwithchildren.vic.gov.au

Please make payment via EFT to: Selby Community House; BSB: 633-000; Acc No: 1231 58065; Reference: (please include your Business/Organisation name). For Credit Card payments please call the office after submitting your application on (03) 9754 2039 during our office hours: Mon-Thu 9.00am-3.00pm and Fri 9.00am-1.00pm.
